Improving your patients' medication compliance can be a challenge, but with the appropriate resources and tools, you can help your patients get on the right path.

The Patient Medication Tracker can help you gain valuable insights into some of the reasons why patients might not be following your prescribed treatment plan.

How to Use the Patient Medication Tracker

  • Download the Patient Medication Tracker by clicking the "Download the Medication Tracker" button below
  • Instruct your patients to fill out their tracker while in the waiting room. Although the form is self-explanatory, a nurse or other staff member should be available to help
  • Discuss their answers during the part of the visit you normally devote to talking about medication inventory or therapeutic lifestyle changes. This resource can help uncover potential barriers to medication compliance by giving patients "permission" to be open and honest in talking about their medications
  • Probe for any disconnects between supposedly adherent behavior and dosing or dosing schedule red flags, as well as for any self-reported non-adherence
  • Talk to your patients about the importance of medication adherence and — together — determine next steps for your patients to work on at home. These might be as simple as keeping medications in plain sight or furnishing proof of eligibility to get patient assistance
